- Relevant lifestyle factors (alcohol, pregnancy potential, OTC use) as applicable.

Monitoring Data
- Vitals, home readings, and labs/imaging required for safe continuation (e.g., BMP, A1c, lipids, LFTs, drug levels).
- PDMP / controlled-substance review if applicable and performed.

Exam
- Focused exam as indicated for the medication indication or side-effect concerns.

Assessment
- For each medication addressed: continue / adjust / discontinue / defer refill with rationale.
- Monitoring gaps identified.

Plan
- Exact refill authorization (drug, dose, quantity, refills) or titration schedule.
