Visit Context
Established patient office visit for fatigue and intermittent palpitations × 3 weeks. Goals: rule out anemia/thyroid issues and discuss whether caffeine is contributing.

Subjective
Fatigue worse afternoons; palpitations lasting seconds, no syncope, chest pain, or dyspnea at rest. Sleep 6 hours; caffeine 3 cups/day. Adherent to levothyroxine. No recent illness. ROS otherwise negative for fever, weight loss, or melena.

Objective
BP 118/72, HR 78, BMI 27.4. Alert, no distress. Heart regular, no murmur. Lungs clear. Conjunctiva pink. TSH and CBC drawn today; prior A1c 5.6.

Assessment
1) Fatigue — likely multifactorial (sleep debt, caffeine); rule out anemia/thyroid under-/over-replacement.
2) Palpitations — low-risk intermittent; no red-flag features today.

Plan
Labs pending. Counsel on sleep hygiene and taper caffeine. Return in 2 weeks with results or sooner for syncope, sustained tachycardia, or chest pain. Discussed NP plan; patient agrees.
