Interval History
- New diagnoses, hospitalizations, surgeries, and medication changes since last preventive visit.
- Reproductive health, sexual health, and safety topics as age/context appropriate and discussed.

History Updates
- PMH, PSH, family history, social history (tobacco, alcohol, substances, occupation, relationships/support).
- Allergies and current medications.

Review of Systems
- Age-appropriate ROS; note positives deferred to problem-focused follow-up if needed.

Exam
- Vital signs, BMI, and exam components performed for preventive visit.

Screenings
- Age- and sex-appropriate screenings: cancer (breast, cervical, colorectal, lung, prostate as indicated), metabolic (BP, lipids, diabetes), bone health, STI, depression, and others per guidelines and patient preference.
- Document completed, ordered, declined, or deferred with reason.

Immunizations
- Vaccines reviewed and administered or recommended today; contraindications/refusals documented.

Counseling
- Nutrition, physical activity, sleep, tobacco/alcohol, sun safety, injury prevention, and other counseling provided.
- Shared decisions for screening choices documented.

Assessment
- Preventive visit completed; chronic conditions briefly noted if relevant to prevention plan.
- Acute issues addressed today (if any) or scheduled for follow-up.

Plan
- Orders placed, referrals, lifestyle goals, vaccine plan, and timing of next preventive visit.
- Patient education materials or after-visit summary items as discussed.
