Realistic example
Patient on escitalopram 10 mg for 6 weeks: anxiety from 8/10 to 5/10, PHQ-9 16→11, residual insomnia, mild early nausea resolved, one missed dose while traveling, no SI. Decision: increase to 15 mg and follow up in 3 weeks.
