Interval History
- Pain, swelling, stiffness, instability, numbness, wound issues, fever, or thromboembolic symptoms since last visit.
- Function: weight-bearing status, assistive devices, ADLs/work/sport participation, and adherence to restrictions/home exercise.
- For post-op: procedure date/type, current post-op week/month, and rehab progress.

Exam
- Wound/incision status if applicable; effusion, ROM, strength, stability, neurovascular exam, and gait.
- Brace/cast/splint condition and skin under device if assessed.

Imaging / Data Reviewed
- Interval X-rays or other studies with healing/alignment/implant findings as discussed.

Assessment
- Interval status (healing as expected / delayed / complication) and working diagnosis update.

Plan
- Changes to weight-bearing, ROM limits, bracing, medications, or PT orders.
- Injections, procedures, or revision/surgical planning if indicated.
- Activity/work notes, DME, and follow-up timing with precautions.

Interval History

Exam

XR knee: tunnels and hardware appropriately positioned; no fracture.

Assessment
Routine 6-week post-ACLR progress; motion and wound healing satisfactory.

Plan
Wean brace for ADLs per PT; continue ACL rehab protocol phase 2. Advance closed-chain strengthening; no pivoting sports. Continue naproxen PRN. Follow up at 3 months with PT progress note; call for calf swelling, fever, or loss of extension.
