Interval History
- Cardiac symptoms since last visit: chest pain/pressure, dyspnea, orthopnea, PND, palpitations, syncope/presyncope, edema, claudication, or fatigue.
- Functional status (NYHA class or activity tolerance) and recent hospitalizations or ED visits.
- Medication adherence, dose changes, and adverse effects (including hypotension, bradycardia, bleeding, electrolyte issues).
- Lifestyle factors discussed: sodium intake, activity, tobacco, alcohol, weight changes.

Data Reviewed
- Recent ECG, echocardiogram, stress testing, cardiac CT/MRI, Holter/event monitor, labs (BNP/NT-proBNP, lipids, electrolytes, INR), and device interrogation findings as discussed.
- Pending or outstanding cardiac studies.

Exam
- Vital signs including BP, HR, SpO2, and weight/BMI when available.
- Focused cardiovascular exam: JVP, heart sounds/murmurs/rubs/gallops, rhythm, lung findings, edema, pulses as relevant.

Assessment
- Active cardiac diagnoses with current clinical status and trajectory.
- Brief reasoning linking symptoms, exam, and data reviewed.

Plan
- Medication adjustments with rationale and monitoring plan.
- Further testing, referrals (e.g., EP, HF clinic, cardiac rehab), lifestyle counseling, and follow-up interval.
- Patient education and return precautions.
