Visit Overview
- Date of visit and clinician/clinic name.
- Plain-language reason for visit and main findings or diagnoses discussed (avoid unexplained abbreviations).

What We Did Today
- Exams, tests, procedures, vaccines, or referrals completed during the visit.
- Results available today in plain language; note results still pending.

Home Care Instructions
- Activity, diet, wound care, symptom management, and work/school guidance as discussed.
- Devices, dressings, or monitoring the patient should continue (e.g., home BP).

Medications
- New medications: name, dose, how/when to take, and common side effects to watch for.
- Changed or stopped medications with brief reason.
- Reminders about continuing home medications unchanged.

Follow-Up
- Next appointment timing and with whom; any labs or imaging to complete beforehand.
- Referrals placed and what the patient should expect (scheduling, prep).

When to Call or Seek Care
- Specific return precautions and after-hours / emergency guidance.
- Clinic contact information for questions.

- Key decisions made together and topics deferred to a future visit.
