Chief Complaint
- Primary reason for urgent care visit and symptom duration.

History of Present Illness
- Onset, mechanism (if injury), course, severity, associated symptoms, treatments tried.
- Red flags and contraindications reviewed for the presenting complaint.
- Relevant PMH, medications, allergies, tetanus status if injury, and last menstrual period if relevant.

Exam
- Vital signs and focused exam; neurovascular status for injuries.
- Document key negatives supporting discharge home when applicable.

Diagnostics / Procedures
- Imaging, labs, POC tests ordered/reviewed.
- Procedures performed (e.g., laceration repair, I&D, splinting) with indication—link to full procedure note if separate.

Assessment
- Diagnosis / working impression and acuity.
- Differential briefly if uncertain.

Disposition / Plan
- Medications, wound care, immobilization, work/school restrictions.
- Return precautions and follow-up (PCP, ortho, ED).
- Transfer/ED referral rationale if escalated.
- Supervising physician discussion if required by site policy.

RICE; air stirrup brace; crutches PRN 48–72h. NSAID PRN. Work note: desk duty 3 days, no ladders. Return/ED for numbness, cool foot, or inability to bear any weight after 72h. Ortho/PCP follow-up 5–7 days if not improving. Patient verbalizes understanding.
