- Therapy attendance and themes if coordinated care is discussed.

Medication Response
- Current anxiolytic/antidepressant regimen, adherence, benefit, adverse effects.
- PRN benzodiazepine or other rescue medication use pattern if applicable.

Mental Status Examination
- Focused MSE including anxiety level, affect, thought content (worries/catastrophizing), and insight.

Safety / Risk
- SI/HI, panic-related catastrophic fears, substance use escalation, and risk formulation.

Assessment
- Anxiety diagnosis status and treatment response (e.g., GAD, panic disorder, mixed anxiety-depression).

Plan
- Medication adjustments, exposure/homework focus, therapy coordination, education, and follow-up timing.
