Group Therapy Documentation: Practical Tips for Therapists Using AI Scribes

Group therapy multiplies documentation complexity: one session, multiple participants, privacy boundaries between members, and payer rules that vary on whether you need a group note only, individual addenda, or both.

AI scribes do not eliminate clinical judgment about what belongs in each client's chart — but they can speed structuring summaries you dictate after group ends.

Types of group documentation

TypePurpose
Group noteSession topic, interventions, group dynamics (de-identified)
Individual addendumClient-specific participation, progress, risk updates
Attendance logBilling and program compliance

Know your payer and agency rules before standardizing a template.

Privacy in group settings

Group notes should avoid identifiable information about other participants in any individual's chart unless clinically necessary and permitted. Document:

  • General session theme and curriculum
  • Your interventions as facilitator
  • Individual member's own statements and behaviors — not other members' confidential disclosures attributed across charts

AI drafts from your dictation — you scrub cross-client identifiers.

Recommended workflow

  1. Facilitate group without in-room recording if that fits your model
  2. Immediately after: dictate 3–5 minute summary — group theme + individual snippets per required chart
  3. AI structures into DAP/BIRP/agency format
  4. Review each note for privacy boundaries before sign-off

SUD and IOP/PHP programs

Intensive outpatient and partial hospital programs often run multiple groups daily. Batch dictation between groups beats reconstructing six sessions at midnight.

When ambient capture is a poor fit

Group room audio picks up cross-talk, side conversations, and overlapping speakers — ambient AI may misattribute speech. Post-group dictation is usually more accurate for facilitators.
