Indication - Clinical indication and relevant pre-procedure findings. Consent & Safety - Risks, benefits, alternatives discussed; informed consent obtained (patient/surrogate). - Timeout / site verification performed as applicable. - Allergies, anticoagulation, and pertinent labs reviewed. Anesthesia / Prep - Local/regional/sedation used with doses; skin prep and sterile technique as applicable. Technique - Stepwise description of the procedure as performed. - Instruments, devices, implants, or suture used. - Specimens obtained and disposition (pathology/culture labels). Findings - Intra-procedure findings relevant to diagnosis and plan. Complications / Tolerance - Complications or “none”; patient tolerance and immediate post-procedure status. Aftercare / Plan - Dressings, activity restrictions, medications, warning signs, and follow-up. - Orders placed (imaging, antibiotics, wound check).
