Interval History - Indication status (controlled / partially controlled / uncontrolled symptoms). - Adherence, missed doses, and barriers (cost, access, side effects). - Adverse effects, allergies/intolerances, and interactions of concern. - Relevant lifestyle factors (alcohol, pregnancy potential, OTC use) as applicable. Monitoring Data - Vitals, home readings, and labs/imaging required for safe continuation (e.g., BMP, A1c, lipids, LFTs, drug levels). - PDMP / controlled-substance review if applicable and performed. Exam - Focused exam as indicated for the medication indication or side-effect concerns. Assessment - For each medication addressed: continue / adjust / discontinue / defer refill with rationale. - Monitoring gaps identified. Plan - Exact refill authorization (drug, dose, quantity, refills) or titration schedule. - Labs ordered before next refill if needed. - Patient education on administration, side effects, and when to call. - Follow-up interval for clinical reassessment.
