ED Discharge Summary Presentation - Chief complaint and brief HPI summary. ED Course - Key exam findings, diagnostics, treatments, reevaluations, procedures, and consultations. - Clinical reasoning and differential considered. Final Diagnoses - Primary and secondary diagnoses at discharge. Condition at Discharge - Clinical status, vital signs stability, and ambulatory/mental status as relevant. Medications - Medications administered in ED (clinically significant). - Prescriptions provided with dosing and duration. - Medications to continue/stop/change if discussed. Discharge Instructions - Activity, diet, work/school guidance, wound care, device care, and medication counseling. - Contagion precautions when applicable. Return Precautions - Specific symptoms that should prompt immediate return / EMS activation. - Expected symptom course and when to seek non-emergent care. Follow-Up - PCP and specialty follow-up timing; referrals or pending results to be reviewed. - Results pending at discharge and how patient will be notified if applicable. Disposition - Discharged home / to facility / AMA / elopement as applicable, with capacity/AMA elements if relevant.

Presentation 28-year-old with 1 day of dysuria, frequency, and mild suprapubic discomfort. No fever, flank pain, or vaginal discharge. ED Course Afebrile, nontoxic. UA positive for leukocyte esterase and nitrites; urine culture sent. No CVA tenderness. Treated with oral fluids and analgesia in ED with improvement. Final Diagnoses Acute uncomplicated cystitis. Condition at Discharge Improved; voiding comfortably; vital signs normal. Medications Nitrofurantoin 100 mg BID × 5 days prescribed. Phenazopyridine as needed for dysuria × 2 days. Continue home medications otherwise unchanged. Instructions / Return Precautions Hydrate; complete antibiotics even if symptoms improve. Return for fever, flank pain, vomiting, inability to void, or worsening pain. Phenazopyridine may discolor urine orange—expected. Follow-Up PCP in 2–3 days if not improving, or sooner PRN. Culture results will be reviewed; patient will be contacted if antibiotic change needed. Disposition Discharged home in stable condition; verbalized understanding of instructions.
