- Current functional impairment at work/school/home.

Collateral / Developmental History
- Academic history, prior evaluations, learning differences, and caregiver/partner observations if available.

Screening / Rating Scales
- Document scales discussed (e.g., ASRS, Vanderbilt) and scores if provided—do not invent scores.

Differential Considerations
- Anxiety, depression, trauma, sleep disorders, substance use, and other conditions that can mimic or co-occur with ADHD.

Mental Status Examination
- Focused MSE with attention, activity level, and organization of thought.

Assessment
- ADHD diagnostic impression (presentation type) and comorbidities.

Plan
- Further testing/collateral needed, behavioral strategies, medication discussion, monitoring, and follow-up.
